Of course, at some point we need to perform hands on treatment. But with this new manner of seeing patients, we can come to the right diagnosis and treatment plan for most patients more efficiently, quickly, and painlessly, with no loss of personal one-on-one communication.” This is a very welcome new era of telemedicine that has been forced on us by the current difficult times.

Dr. Silber was invited by Chinese medical experts to export his technology for ovary freezing and transplantation, and preservation of fertility for cancer patients to China, as well as to preserve the biological clock to protect women from becoming infertile just by aging. These are photographs from his historical mission to China in Guangzhou in February 2012, where he performed the first ovary transplant and ovary freezing in China, and taught Chinese doctors from all over the country. Nanfang Metropolis Daily February 10, 2012 Recently, a case of ovarian tissue transplantation was completed in Guangzhou, led by a world famous fertility surgeon from the United States. This was the 10th case of its kind world-wide, and is the 1st such ovarian transplant in China. Ovarian tissue was donated from one twin sister to another, and the recipient twin can now expect to resume ovulations and conceive naturally. The surgeons who completed this transplant were world-renown experts on reproductive medicine and fertility preservation, Dr. Sherman J. Silber, and Dr. John Zhang from US, and Prof. Xiaoyan Liang from the Sun Yat-sen University Medical School in Guangzhou, China. The operation for the donor and recipient lasted for a total of 3 hours, and was minimally invasive. Both the donor and recipient can resume normal sexual intercourse 1 week after the surgery. For all of the 9 previous cases completed by Dr. Silber among monozygotic twins discordant for POF showed, menstruation always resumed in about 90 days’ time, along with resumption of normal ovulatory cycles and natural conception. Since the twins monozygotic (identical) by STR detection techniques, transplantation of ovarian tissues between them will not incite any immune rejection, which means that no anti-rejection agents will be needed. The first case of ovarian tissue transplantation between monozygotic twins discordant for POF was reported in 2005 by Dr. Silber from St. Luke’s Hospital of St. Louis, Missouri in the USA. Such operations were completed between another 8 pairs of twins by Dr. Silber before the afore-mentioned Chinese case performed by Dr. Silber. Of the 9 cases in US, all recipients resumed normal endocrine profile with ovulation resulting in 11 healthy babies. This technique can be used to preserve the fertility of young cancer patients, and also protect all women from the aging effects of their biological clock. Dr. Silber now in China has completed his 10th case of ovarian tissue transplantation [technical video] between monozygotic twins. As he shook hands with the recipient on the next day after the surgery, he smiled and told her that the only thing that she needs to do would be to have sex with her husband. Things can again become simple, because within 63-100 days, menstruation of the recipient would resume, and the approach is so robust that there has even been 3 cases by Dr. Silber in which the recipient achieved 3 natural conceptions and delivered 3 healthy babies as late as 7 years later, all from the same transplanted ovary tissue. Dr. Silber has talked about some of the advantages of ovarian tissue transplantation over oocyte donation at the press release conference. Firstly, it endows the recipient the opportunity to conceive completely naturally again, with complete restoration of endocrine function. Secondly, since hundreds of thousands and millions of eggs can be retrieved with ovarian tissue transplant, the pregnancy prospect of transplantation is much more robust than oocyte donation. Notably, the operation does not cause any adverse effect on the donor’s ovarian reserve and time for menopause. Technically speaking, this ovarian tissue transplantation technique can be used to preserve fertility for normal individuals and cancer patients. For cancer patients,notably, with careful pathological sampling and examination to exclude ovarian cortex cancer involvement, ovarian cortex removal, cryopreservation and auto-transplantation is very safe. Moreover, since recovery from the operation will take only just 1-2 days, the cancer patients can move on with their chemo- or radio- therapies as planned, with no delay. Dr. Silber and other experts on fertility preservation world-wide have so far performed ovarian tissue banking for a series of patients suffering from diseases like breast cancer, Hodgkin’s lymphoma , Ewing’s Sarcoma, haematological malignancies or neurological diseases that require bone marrow transplantation. 18 healthy babies have been conceived and delivered from cancer patients with the help of ovarian tissue cryopreservation and auto-transplantation. Prof. Liang, with Dr. Silber and Dr. Zhang’s partnership, now looks forward to the establishment of the first fertility preservation centre nation-wide in China.
